Our callback form asks only for a parent or guardian name, phone number, optional email address, and preferred contact time. Do not use the website form or a general email inbox to send diagnoses, medications, insurance details, patient records, or other health information.

We use website contact information to respond to enrollment questions, arrange a call, and provide next-step guidance. The callback form is processed by FormSubmit and delivered to Elite PPEC by email. Information may also be handled by service providers that support website hosting, analytics, and communications.
Patient records and referrals
Providers and families should call (754) 263-2925 before sending records. Our intake team will provide the current approved submission method. Do not email patient records or protected health information to a general Elite PPEC inbox.
