How to refer a patient

Four steps. Most referrals turn into intake assessments the same day we receive them.

  1. 1

    Send the referral

Fax discharge summary, current H&P, active medication list, current orders, and insurance information to (754) 263-2925 or email a secure summary to don.eliteppec@gmail.com. A signed physician prescription for PPEC is required — we can help obtain one if needed.

  2. 2

    One-business-day response

Our intake nurse reviews every referral within one business day and contacts the discharge planner or case manager directly to confirm clinical fit and schedule an intake assessment.

  3. 3

    We handle the Medicaid PA

Elite PPEC's intake team handles all Florida Medicaid prior-authorization paperwork on behalf of the family. Typical PA turnaround is 7–14 days. We follow up with the MCO so the family doesn't have to.

  4. 4

    Smooth discharge-to-PPEC transition

On approval, we coordinate transport, confirm the care plan with the physician, and can begin services the same week. Nursing notes are shared back with the referring team.

Provider FAQ

Which Florida Medicaid MCOs does Elite PPEC work with?
Elite PPEC bills Florida Medicaid directly and works with all major Florida Medicaid Managed Care plans, including Sunshine Health, Simply Healthcare, Molina Healthcare, Aetna Better Health, UnitedHealthcare Community Plan, Community Care Plan, and Humana Healthy Horizons. If you're unsure about a specific plan, call us and we'll verify on the same call.
How quickly can a child start after discharge?
Once prior authorization is in place, children can typically start within 1–3 business days. In urgent discharge cases, we prioritize intake to avoid holding up the discharge.
Do you accept ventilator-dependent children?
Yes. Elite PPEC is staffed and equipped for ventilator management, tracheostomy care, and complex respiratory support. Our respiratory therapist and RN team coordinate directly with your pulmonology service.
What is the nurse-to-child ratio?
Elite PPEC maintains a 1:5 nurse-to-child ratio with additional therapy and support staff on-site. Higher-acuity children receive 1:1 or 1:2 attention as clinically indicated.
Can Elite PPEC coordinate home health wrap-around hours?
Yes. We routinely coordinate PPEC coverage alongside home health private-duty nursing hours so families have full-day coverage across settings.
Do you provide transport?
Yes — free door-to-door medical transport in specialized vehicles equipped for children with mobility and medical equipment needs. Transport is covered by Medicaid and included at no cost to the family.
